You cannot simply amend your existing passport after a divorce. HM Passport Office (HMPO) requires a full passport renewal — which means paying the full fee and losing any remaining validity on your current passport. No carryover of unused years.

The Exact Document Package
HMPO is strict about what they accept. Submit all of the following:
- Completed application — online is faster and cheaper
- Your current passport — it will be physically cancelled and returned to you
- Your original birth certificate — showing your birth surname
- Your original marriage certificate — showing both your maiden and married names
- A signed personal statement declaring: "I have returned to my maiden name for all purposes, and I will no longer use my married name at all." This must be handwritten or typed and signed by you
- One piece of official evidence of the new name in use — a utility bill, council tax letter, payslip, or letter from your local authority in your restored surname
You do not need to submit the Final Order itself for a maiden name reversion. The marriage certificate plus the personal statement is sufficient. However, if you are changing to a completely new name via deed poll, you must include the deed poll instead.
Common Rejection Reasons
Printed email copies of the Final Order. HMPO does not accept screen-printed PDFs of the digital Final Order as a "certified copy." If you need the Final Order as evidence (for a deed poll name change), order a certified paper copy from HMCTS first — around £11.
Missing personal statement. The signed declaration that you have abandoned your married name is mandatory. Without it, the application is returned.
Photo rejected. Digital photos submitted online are checked automatically. The most common failures are shadows, glasses reflections, and the head being too large or small in the frame.
