Why You Need to Separate Your Medicare Card
While you and your ex-spouse share a Medicare card, either of you can access the other's claiming history, pharmacy records, and safety net balances through myGov. Beyond the privacy concern, a shared card can also create confusion around who has reached the Medicare Safety Net threshold, which affects out-of-pocket costs for the rest of the calendar year.
Separating your Medicare card is a straightforward process, but it needs to happen in the right place in your post-divorce update sequence — after your driver licence is updated, so you have current photo ID to present.
How to Get Your Own Medicare Card
You need to complete the "Application to copy or transfer from one Medicare card to another" (Form MS011).
You can do this:
- Online through your myGov account linked to Medicare
- In person at a Services Australia service centre
- By phone on the Medicare general enquiries line (132 011)
The form asks you to nominate which individuals should be on your new card. You will receive a new card number — your old card remains active for your ex-spouse and any other members listed on it.
Processing typically takes 5–10 business days, though the online route is often faster.
What About the Children?
Children under 15 can be copied onto both parents' Medicare cards. This ensures both parents can claim Medicare benefits for the child regardless of who takes them to the doctor.
However, removing a child under 15 from a parent's Medicare card requires the written consent of both parents. If you cannot obtain your ex-spouse's consent — for example, due to domestic violence or a complete communication breakdown — you will need to provide Services Australia with a certified copy of a court order that grants you sole parental responsibility, or a family violence protection order.
Children aged 15 and over can be transferred to their own Medicare card if they choose.

Updating Your Name on Medicare
If you are reverting to your maiden name, you can update your Medicare name at the same time you separate your card. You will need to provide linking evidence — your birth certificate, marriage certificate, and divorce order — to show the connection between your current name and your previous name.
Update your driver licence first. Services Australia will accept your updated Queensland driver licence as primary photo ID for the name change, which simplifies the process significantly compared to bringing multiple original documents.
Medicare Safety Net Implications
The Medicare Safety Net accumulates your out-of-pocket gap costs over the calendar year. Once you hit the threshold, Medicare covers a higher percentage of costs for the rest of the year.
When you were registered as a family, your ex-spouse's medical expenses may have contributed to the family threshold. After separation, tell Services Australia about the change and check your Medicare Safety Net registration; the treatment of your registration and accumulated costs depends on the updated family circumstances. If you have dependent children on your card, ask Services Australia how to register the appropriate family arrangement.
If you separated partway through the year and have already accumulated significant gap costs, ask Services Australia how those costs will be treated after your Medicare Safety Net registration changes.
Private Health Insurance Is Separate
Medicare separation and private health insurance separation are two completely different processes. Splitting a family private health policy into individual policies is handled directly with your insurer — Medicare has no involvement. If you are on a family policy, contact your insurer separately to discuss converting to an individual policy and the premium implications.

Medicare should be updated after your driver licence and passport, but before you notify banks and super funds. The recommended sequence:

- Update your Australian passport
- Separate and update your Medicare card
