You can download every Arizona divorce form for free from your county's Superior Court website or the Arizona Judicial Branch self-service center. The problem is not getting the forms — it is knowing which ones you actually need, what order to file them in, and which ones your county requires that other counties do not.

Core Filing Forms (All Counties)

Every Arizona divorce begins with the same set of documents, whether you are in Maricopa, Pima, or a rural county:

Petition for Dissolution of Non-Covenant Marriage — The primary filing document. States your residency, date of marriage, community property and debts, and what you are requesting (property division, custody, spousal maintenance). Use the "with minor children" version if you have kids under 18.

Summons — Notifies your spouse that a divorce action has been filed. It warns that failure to respond within 20 days (30 days if served out of state) will result in a default judgment.

Preliminary Injunction — Takes effect automatically when you file. Prohibits both spouses from selling, hiding, or transferring community property, taking out new loans against shared assets, or removing minor children from Arizona without consent or a court order.

Sensitive Data Cover Sheet — Contains Social Security numbers, dates of birth, and contact details. This form is sealed by the court and is never served on the other party.

Notice of Health Insurance Coverage — Required disclosure about existing medical insurance.

Notice to Creditors — Statutory notice regarding joint debt obligations.

Forms for Cases With Minor Children

If you have children under 18, you will also need:

Affidavit Regarding Minor Children — Lists every address where each child has lived for the past five years. Required under the UCCJEA to establish custody jurisdiction.

Parenting Plan — Your proposed custody and visitation schedule. Arizona uses "legal decision-making" (custody) and "parenting time" (visitation) terminology.

Child Support Worksheet — Calculates guideline support based on both parents' incomes and parenting-time allocation.

Forms for Finalizing Your Divorce

The closing documents depend on your path:

Consent Decree of Dissolution — Used when both parties agree on all terms. Must divide every community asset and debt, address spousal maintenance, and incorporate the signed Parenting Plan and Child Support Worksheet (if children are involved).

Application and Affidavit for Default — Filed when the respondent does not respond within the 20-day window after being served.

Default Decree of Dissolution — Submitted after the default application is granted and the 10-court-day grace period expires.

Resolution Statement — Required in Maricopa County before the judge will sign a Consent Decree; confirms that all financial disclosures have been exchanged.

Where to Get Arizona Divorce Forms

Maricopa County — The Law Library Resource Center at superiorcourt.maricopa.gov provides complete packets organized by case type (divorce with children, divorce without children, legal separation). Instructions are included in each packet.

Pima County — The Self-Service Center at 110 West Congress Street in Tucson has printed packets. Forms are also available online.

All counties — The Arizona Supreme Court Self-Service Center (azcourts.gov/selfservicecenter) hosts standardized forms accepted statewide. However, some counties have additional local forms or cover sheets.

The Gap Between Free Forms and a Finished Filing

Free court forms give you the raw documents, but they do not tell you the sequence of steps, the county-specific filing fees, or the deadlines that govern when each form must be submitted.
